As the relevant person (data subject), in accordance with the provisions of Article 13 of the PDP Law No. 6698 and the "Communiqué on Procedures and Principles of Application to the Data Controller" dated 10.03.2018 and numbered 30356, you can send your requests regarding your rights, specified in Article 11 of the same law and listed in Article 9 below, in writing to our company’s head office address (Acıbadem Mah. Tekin Sok.No:5 (Havuzlar Sitesi Köşkü) Acıbadem | Istanbul - TURKEY) or to our registered electronic mail (KEP) address “wcep@hs01.kep.tr”.
In your application, the following must be present:
Name, surname and signature if the application is written;
TR ID number for citizens of the Republic of Turkey; nationality, passport number or identification number, if any, for foreigners;
Residence or workplace address subject to notification;
E-mail address, telephone and fax number for notification, if any;
Subject of the request;
as per article 5/2 of the above-mentioned notification, and unfortunately, applications that do not contain one or more of this information cannot be processed in accordance with the legislation.
The requests you can submit within the scope of Article 11 of the PDP Law are as follows:
1- You can learn whether your personal data is processed or not;
2- If your personal data has been processed, you can request information about it;
3- You can learn the purpose of processing your personal data and whether it is used in accordance with the purpose;
4- You can find out the third parties to whom your personal data is transferred, at home or abroad;
5- If your personal data is incomplete or incorrectly processed, you can request their correction;
6- You can request that your personal data be deleted or destroyed;
7- You can request that your correction, deletion or destruction requests be notified to third parties to whom personal data has been transferred;
8- If a result arises against you by analyzing your processed personal data exclusively through automatic systems, you can object to this result;
9- If you suffer damage due to unlawful processing of your personal data, you can request compensation for the damage.
